Appointments

Our practice is primarily by appointment only, however we do accept walk-ins if the schedule allows it for sick visits.  Appointments for new patients and physicals can usually be made within a few weeks.  Please call our appointment desk at 

(615) 826-3101 to schedule an appointment.

New Patients

The day of your first visit please arrive 15 minutes before your scheduled appointment time.  You'll need to bring your current insurance card(s), a driver's license or photo ID, and all of your current medication(s)/supplement(s) to your appointment.  Please note that ALL copays are due at the time of service.

Returning Patients

Please arrive 10 minutes prior to your scheduled appointment time, with your current insurance card(s), a driver's license or photo ID, and have your copay ready (if applicable).  Be sure to notify the front desk of any insurance, phone number, or address changes.  While this may seem like a nuisance, we have found that even slight changes can make it difficult to file your claim with the insurance company.  A small inconvenience now saves a larger headache down the line.

No Show Policy

Failure to fulfill office appointments "no shows" will be recorded in your record. Breaking an appointment hinders our ability to care for you as well as others, since we lose a time slot that could have been used to help another patient.  Barring any unusual circumstances, if a patient fails to show for more than two appointments in a three month period, he or she may be dismissed from the practice.

Running Late

In order to give you and your health the time you deserve, you may be asked to reschedule if you arrive more than 15 minutes later than your scheduled appointment time.  If possible we will try to accomodate you with another time slot close to when you arrive.  Unfortunately, this can't always be done while still honoring the appointments of others.

Likewise, your time is equally valuable.  We sometimes run behind schedule when one of our patients presents with an unexpected complication or situation.  At those times, our staff will make every effort to see you on time and will notify you promptly of any unavoidable delay. You may decide to reschedule, wait to be seen, or see a different provider than you were originally scheduled with.

Can I bring my children to the appointment?

We have several books and toys in the waiting room to keep your child entertained, but they may NOT be left unattended while you are being seen.  Your child may bring a few of these back to the room with them during your appointment.
